I'd like to install some software on an older 2003 server and monitor
all unique IP addresses that access that server. Be even better if I
could tell if they were using IP vs FQDN.
We're in the process of moving an older DB server to a new VM, but want
to make sure we know what is pointing to it so we can be ready or make
the changes needed.

You will need to install the program with the Agent service (+WinPcap driver included) locally on that server and create a traffic sensor of the "sniffing packets" type. The sniffer will collect IP addresses of connecting hosts and you will be able to create a report on these addresses.
